Many serious accidents have occurred because an equipment started running when it was supposed to be shut off to prevent repairs or maintenance. These accidents are usually caused by energy that was not released correctly. Lock-out-tag-out (LOTO) which makes sure that machines are completely off until a person with authorization releases them, helps avoid these incidents.
The LOTO system consists of a number of locks, tags, and other devices used to secure, isolate the machine from electricity. All "affected" and "authorized" personnel are required to be trained in the use of these lockout/tag-out devices. These devices include, but aren't limited, to chains, locks, and padlocks. Each device is identified and labeled, containing the date and time of application, the name of the authorized individual, and the reason for the lockout/tag-out (repair, maintenance, etc). It is imperative to state clearly that these devices will not be removed until the LOTO procedure is complete.
